You have the right to access your health information. Use the form below to request a copy of your records from CompDrug.
Which Type of Authorization Do You Need?
Release of Information (ROI)
Use this when you want CompDrug to send your health information to another person or organization.
Examples:
- Attorney
- Probation officer or court
- FCCS or another agency
- Family member or support person
You choose who receives the information and what information CompDrug is allowed to release.
Exchange of Information
Use this when CompDrug and another provider or organization need to share information with each other.
This may be helpful when:
- Two healthcare providers need to communicate about your care
- Information needs to be shared in both directions
- Ongoing coordination is needed between CompDrug and another provider
An exchange allows both parties to share the information you authorize, rather than only sending information one way.
Treatment, Payment, and Operations (TPO)
Your TPO consent allows CompDrug to use and share information for treatment, payment, and normal healthcare operations.
TPO may be used for:
- Providing and coordinating your treatment
- Billing and payment
- Healthcare operations such as quality improvement and care management
TPO does not cover every request. If you want information sent to a probation officer, court, FCCS, family member, attorney, or another outside person or agency, you may need a separate ROI or Exchange of Information.
What Happens After I Submit My Request?
Submit your request
Complete the Medical Records Request Form and tell us which records you would like for yourself.
We review your request
Medical Records may contact you if additional information is needed to identify the records you requested or verify your identity.
We prepare your records
Requests are processed as soon as reasonably possible. Under HIPAA, requests generally must be acted on within 30 calendar days. In limited circumstances, an additional 30 days may be permitted if you are notified in writing of the reason for the delay and the expected completion date.
You receive your records
Your records will be provided using the delivery method arranged as part of your request.
Complete Your Authorization Online
The fastest and easiest way to start your request is to complete the electronic authorization form.
- Your signature is captured electronically.
- Your request is routed automatically to the appropriate staff.
- No additional action is needed from you unless we contact you for more information.
Before you start: Have the name and contact information for the person or organization you want to share information with.
Need Help or Unable to Complete the Form Online?
If you are unable to complete the electronic form, a Care Coordinator can help you prepare a paper authorization.
Your signature is required before a paper authorization can be processed. You will need to come to CompDrug and sign the completed form before we can act on your request.
Completing the electronic form avoids this extra step because you can sign and submit your authorization online.
Your Records Have Special Privacy Protections
Because CompDrug provides substance use disorder treatment, many of our patient records are protected by a federal confidentiality law known as 42 CFR Part 2, in addition to HIPAA.
Part 2 provides special protections for records related to substance use disorder treatment. Federal law limits how these records may be used and disclosed. Part 2 also provides additional protections related to the use of these records in certain legal proceedings.
These privacy protections do not prevent you from requesting access to your own health information.
